What a difference a dental implant can make…

Dental implant surgery is a particular focus for my professional work.

The benefits achievable with dental implants can be key to effective cosmetic dentistry and restorative dentistry work – all playing part of aesthetic and functional improvements (also more commonly known as a ‘smile makeover’)…

There can be a number of reasons why a dental implant can be the effective solution for a patient.

Implants can make significant difference in function and appearance.

I recall looking after one male patient from our Glasgow dental clinic who had suffered a sports injury – he’d been a footballer and in a clash had instantly knocked out his two front teeth. I was able to see him at short notice and assess the damage caused in the accident.

Like anyone the loss of these teeth and the impact on his smile were fairly traumatic factors but it was evident to me on examination that we could achieve a long-lasting and effective solution for the patient by using dental implants.

The treatment took about 15 months in total but during the patient journey we kept him fully informed with what was happening and what needed to happen to achieve the end result.

Much of that time is taken up in the simple healing process post surgery and to allow the titanium implant to become properly secured in the patient’s mouth.

The patient’s own words to us in a testimonial summed up the outcome following completion of his treatment plan.

All the above are sound reasons for visiting your dentist for a check-up but just how good is our country when it comes to visiting the dentist?

Recent survey results issued by the British Dental Health Foundation revealed that the UK is one of the most likely nations in Europe to visit their dentist for a check-up – ranked second (72%), after the Netherlands (79%).

At our dental clinic here in Glasgow the dental professionals recommend you visit for a check-up twice a year/every six months.

The Foundation’s survey found that when it came to reasons for visiting the dentist –

27% of adults only visit their dentist when they have a problem

9% of adults visit their dentist with pain

90% of visits to the dentist are for check-ups

The survey also found that teeth/smiles are rated the second most important attraction feature, after personality, and the most important body feature surveyed (including body shape, height, hair, face and eyes).

We also recommend regular visits to our hygienist team to ensure you maintain good cleaning standards to ensure all round oral health and maintenance.

Implant retained denture improves patient’s smile

Regular readers of our blog will know that from time to time we highlight the treatment of a patient who’s undergone cosmetic or restorative dentistry work.

In this post today we look at the use of dental implants to retain a denture for a patient at our Glasgow dental clinic.

This 45 year old patient presented wishing to improve the stability and aesthetics of her upper arch.

The patient was suffering from generalised adult periodontal disease, with advanced bone loss around the upper teeth leading to tooth mobility, splaying, tooth rotation, poor aesthetics and functional instability.

EASY TO MAINTAIN

The patient wished an improvement in the look and function of the teeth but required a solution that was easy to maintain and clean going forwards.

As part of the smile makeover process the patient underwent extensive examination and at all times was appraised of what was happening and a detailed treatment plan was given.

A decision was made to remove the failing upper teeth and allow the bone to heal. A conventional denture was placed immediately after these extractions and after adequate healing, four implants were placed in the upper arch and allowed to heal.

After healing of the implants, a milled bar with locators was fitted to the implants and this was then used to retain an upper denture firmly in position. This solution allowed a very stable upper denture even in function, with vastly improved aesthetics. As the denture is removable, both the denture and the bar/implants are ease to clean and maintain.

MORE NATURAL FEEL

This bar gives great support and retention to the denture and as a result of this, the denture extent/coverage can be reduced meaning that the upper denture does not cover the palate allowing a more natural feel in the final result. The total treatment time for this case was 10 months.

It’s a warm welcome to our newest team member who joins us today…

We’re delighted to welcome Imran Shafi, specialist orthodontist, to the Glasgow dental clinic team of clinical professionals.

Imran joins fellow specialist orthodontist Andrew McGregor in the Philip Friel Advanced Dentistry staff line-up.

Orthodontics can play a vital part in a patient’s aesthetic and functional rehabilitation treatment plan and it’s great having Imran on board to complement and enhance our existing team.

Imran is a graduate from Glasgow University and after completing his vocational training in Newcastle headed to Australia where he worked for two years.

On returning to these shores he completed his specialist orthodontist training, being awarded a Doctorate in Clinical Dentistry (Orthodontics) at Glasgow in 2011.

His research won two extremely important and prestigious prizes – the Royal College TC White Lecturer Award and the Royal Society of Medicine Odontology President’s Prize Award.

I know from speaking with Imran as he prepared to join the team how much he is looking forward to seeing patients here at the clinic.

As with all our dental professionals here, Imran Shafi is registered with the General Dental Council and his registration number is 79325.

Why Are Dental Implants so Good for Replacing Missing Teeth?

If you are missing one or more teeth then the team of dentists at Philip Friel Advanced Dentistry in Glasgow may suggest dental implants as a means of replacement. But why should you consider dental implants, and why are they so good?

Dental Implants are better for Your Oral Health.

Natural teeth help to keep the surrounding bone strong and healthy, as the forces produced during eating are transmitted down the root into the bone. This prompts the bone to continue producing new cells. When a tooth is extracted this stimulation is lost, and the bone will no longer be prompted to maintain its growth and turnover. The bone in the jaw will gradually resorb, and the bony ridge that once held the teeth will become flatter and shallower. This can make it difficult to retain a denture, or could mean that teeth adjacent to the gap will become less stable.

After the necessary examination and assessment, your dentist in Glasgow (Philip Friel can also look after dental implant patients in Edinburgh at the New Town Dental Care clinic) may suggest dental implants to replace missing or failing teeth to improve the function and stability of the dentition.

Dental implants function in a way that is very similar to a natural tooth root, and this is why they are often considered to be the gold standard for replacing teeth. They will help prevent the jawbone from being resorbed heloing to maintain the support for the facial soft tissue overlying.

Dental Implants Don’t Negatively Impact the Remaining Teeth

The alternative methods of tooth replacement are to have a full or partial denture, or to have a bridge fitted by your dentist. A bridge is a popular method of replacing teeth, and it can produce very nice results, but one thing to bear in mind is that it does impact the teeth adjacent to the gap. A bridge consists of one or two crowns that are placed on the teeth adjacent to the gap. These are used to support the replacement tooth which is called a pontic. The pontic, or pontics, are attached to the crowns, providing a stable restoration. However the teeth that are crowned will need to be prepared or ground down, and once this healthy tooth structure is lost it can only ever be replaced artificially. In some cases, where these adjacent teeth are already filled or crowned, then the bridge option may be more suitable as it will require less loss of healthy tooth tissue.

A partial denture is held in place with clasps that are fitted around the remaining teeth. These teeth may need to be modified to hold the clasps, and the clasps can place additional stresses on the teeth. In contrast dental implants placed by your dentist will not require any preparation or stress on the adjacent teeth.

Implants can make significant difference in function and appearance.

A check-up is essential as you do need to be in good oral health, and any existing dental problems will need treating before implant surgery can be scheduled. You also need to be in FAIRLY good general health, as some medical conditions may impact on the success of dental implant treatments.

Do You Have Sufficient Bone?

For successful Dental implant surgery in Glasgow and Edinburgh, you will require to have adequate bone to retain the implant(s).

STRONG AND STABLE UNION

This is important as it will enable your new implant to integrate with the bone (known as osseo integration), creating a strong and stable union that can support your crown, bridge or denture. Not everybody will have enough quality bone, especially if they lost teeth a long time ago.

This is because the bone gradually reabsorbs after a tooth has been extracted, as it no longer receives any force stimulation from the tooth root. Lost bone is something that can be replaced through having a bone graft. If you do need bone grafting then it is certainly nothing to be concerned about, as even through it sounds dramatic, it is a fairly straightforward, routine procedure.

Gaining new techniques in dental implants for our patients

Dental implants are my particular focus in dentistry and I’ve travelled all over the UK and the rest of the world in pursuit of the latest techniques and understanding of what is, I believe, one of the greatest advances in cosmetic and restorative dentistry in the last 40 years.

For many years now, we have been using the all on four technique with Nobel Biocare implants to restore the dentition in those who have lost or are loosing all of the teeth in one arch.

Recently I travelled to Essex to spend two days with fellow implant dentist Andrew Moore who uses a different implant system to carry out this technique with many benefits that I wanted to see first hand.

Since qualification in 1988 Dr Moore expanded his clinical experience by taking the post of Honorary Clinical Assistant in Oral Surgery at Basildon Hospital. He then completed a 2 year Postgraduate course in Orthodontics at Southend Hospital.

Like me, Andrew’s main interest is in the field of Implantology, which he has pursued with a commitment to postgraduate qualifications in this field, including a 1 year course in Implantology at The Centre for Reconstructive Surgery, Harley Street, London.

SUPERB OPERATOR

He was one of the first dentists to be awarded The Diploma in Implant Dentistry by the Faculty of General Dental Practitioners at The Royal College of Surgeons. Andrew has now been placing dental implants since 1997 has extensive experience and takes referrals for implant treatment from other dentists in the area. Andrew now teaches Implant Dentistry to other dentists at the practice and at the Royal College of Surgeons.

It was good to see Andrew at work – he’s a superb operator – and interesting to see another clinic in full swing, especially when dental implant surgery is underway.

Dental makeover lets patient ‘smile with confidence’

Dental function as well as appearance can be compromised if a patient suffers from dental problems – especially if they’ve started back in childhood and continued into adulthood.

We recently saw a patient who was referred to us from her own dentist. Her issues really began when she was aged just 10 and from her recollection she explained she’d had a lifetime of dental challenges.

I have attended the same dental practice for over 40 years and had as much treatment as my own dentist
could give me. My back teeth were heavily filled ,crumbing and moving. I had a crown at the side that
kept falling out and a dark line showed at the top and another lower crown that was causing me extreme
pain.

GAPS MADE EATING VERY DIFFICULT

I had a few lower back teeth extracted in my 20′s and had gaps there which made eating very
difficult. My front tooth had a veneer on the half tooth I had chipped and the tooth next to it was a different
shape and much slimmer so my smile was squinty looking. I had it all going on with having teeth missing
at the back ,crowns ,veneers and I was really struggling to eat. Worse still at the age of 51, I was in
constant pain with my back teeth, particularly at night.

I had an in depth discussion with my own dentist and he referred my to Dr. Friel as he felt he was the
dentist to sort out the difficulties I was having with my back teeth. he told me that there was no “quick fix”
for me and that I had to be prepared for perhaps over a year of treatment . I trust my own dentist
implicitly. He has been in the business for over 30 years and is highly regarded in the dentistry world so
his recommendation to Dr. Friel was testament enough for me.

A FEELING OF TRANQUILITY

I was apprehensive for my first visit to Philip Friel’s Clinic. You get used to the one dentist and I felt my
teeth were so bad and was embarrassed to have someone else looking in my mouth. However I needn’t
have worried. From the minute I stepped through the front door I was put at ease by Karyn and Siobhan.
The surroundings and decor are calming giving a feeling of tranquillity.

Dr. Friel was amazing from my first visit onwards. At first I was only there to sort out my back teeth lower
and upper. My plan detailed everything and I knew exactly what was going to happen over the coming
months. A year or so later and my back teeth were perfect. I had my smile widened which took away from
the prominence/overbite of my 6 front teeth and they were all perfect in size. Throughout the year I had
root treatments, an extraction, crowns and a partial cobalt chrome partial fitted ( in the coming years I will
get implants ) but for now I have a full set of top and bottom teeth. That’s a first since my early 20′s. Pain
free sleeping is a real bonus.

GREAT COMFORT

I visited the clinic many times over the year and I actually got to like my visits. I always had a chat with
Karyn and Siobhan who were great with me and they too could explain about different treatments. Root
treatments and an extraction with Ross and Louise was virtually painless and I really dreaded that. Every
time I was in the chair with Dr. Friel I felt very at ease and suffered virtually no pain. Kasia and Pam were
a great comfort.

I was so pleased with my back teeth I decided to get Dr. Friel to draw up a plan for my 6 top front teeth
which this time was more cosmetic work than my back teeth had been. Again I knew exactly what was
going to be happening via my dental plan. I completed that treatment last week and the results are
stunning. That was really painless and that work took about 4 weeks over 4 visits. The amazing thing is
that my teeth look so natural and the changes were so gradual that it really doesn’t look like I’ve had my
“teeth done”.
